By accessing, booking, or using our website, online forms, or in-studio services, you agree to the following Terms and Conditions. Please review them carefully before scheduling or paying for any service.

1. Company Overview

Body of Soul Lymphatic Massage Therapy, LLC is a Michigan-based wellness company specializing in lymphatic massage, post-operative recovery, and holistic wellness services performed by licensed and insured practitioners.

All BOS LMT services are non-medical and designed for relaxation, recovery, and overall wellness. They are not substitutes for medical examination, diagnosis, or treatment by a physician.

2. Eligibility

By booking or receiving services, you confirm that:

• You are at least 18 years of age, or you have written consent from a parent or legal guardian.
• You have disclosed all relevant medical and surgical history.
• You understand that BOS LMT practitioners do not diagnose, prescribe, or treat medical conditions.

3. Booking and Payment Requirements

• All services must be paid in full at the time of booking to confirm and reserve your appointment.
• BOS LMT accepts credit/debit cards and other approved digital payment methods.
• All costs for services are final, non-transferable, and non-refundable.
• Prepaid sessions, memberships, and gift certificates may not be exchanged for cash.
• Failure to remit full payment in advance will result in appointment cancellation without notice.

4. Cancellation, No-Show, and Payment Policy (Strict Enforcement)

Payment Requirement

All appointments must be paid in full at the time of booking. Payment confirms your reserved appointment slot. All service fees are final, non-transferable, and non-refundable.

Rescheduling and Cancellation

• Clients must provide a minimum of 24 hours’ written notice (email, text, or through the BOS LMT scheduling system) to cancel or reschedule.
• Cancellations made within less than 24 hours of the appointment time incur a $100 late-cancellation fee, automatically charged to the payment method on file.
• Clients who cancel within the required window may apply their prepaid service once to a rescheduled appointment if booked within 30 calendar days. After 30 days, the prepaid fee is forfeited.
• BOS LMT does not issue refunds for any cancellations, schedule changes, or unused services.

No-Show Policy

• Failure to appear for an appointment without notice is a No-Show and results in forfeiture of the full session fee.
• Repeat No-Shows or excessive cancellations may result in permanent booking restrictions at BOS LMT’s discretion.

Late Arrival Policy

• Clients arriving more than 10 minutes late may have their session shortened to avoid impacting subsequent clients.
• Clients arriving 15 minutes or later after their scheduled time will be marked as a No-Show, and the full service amount will be forfeited.

Emergencies

BOS LMT understands that genuine emergencies may occur. In such cases, written documentation (medical, accident, or comparable proof) must be submitted within 48 hours of the missed appointment for review. Approval of exceptions is solely at management’s discretion and does not guarantee rescheduling or refund.

Scheduling Integrity

Each appointment time is reserved exclusively for one client. Missed or late sessions prevent other clients from receiving care. Strict enforcement of this policy ensures fairness, maintains practitioner schedules, and preserves the quality of service.

5. Refund Policy

All fees for services, gift certificates, or packages are final. No refunds are issued for cancellations, late arrivals, or dissatisfaction with results.

BOS LMT reserves the right to issue store credit solely at management’s discretion in exceptional circumstances.

6. Health and Medical Disclaimer

Massage therapy services are for general wellness and comfort only. Practitioners do not diagnose medical conditions, prescribe medication, or provide medical treatment.

Clients are responsible for consulting a licensed physician regarding any medical concerns. BOS LMT is not liable for adverse reactions or outcomes related to the receipt or application of massage therapy services.

7. Client Responsibilities

Clients must:

• Arrive 15 minutes before their scheduled appointment for preparation and check-in.
• Disclose relevant health information, medications, and surgical history.
• Follow all pre- and post-treatment instructions.
• Maintain appropriate hygiene and behavior.
• Refrain from inappropriate or sexual conduct; such behavior will result in immediate termination of the session and forfeiture of all fees.

8. Practitioner Conduct and Independence

All therapists and subcontractors represent BOS LMT as independent contractors. They are responsible for maintaining proper licensure, insurance, and professional conduct. BOS LMT is not liable for any contractor’s actions outside approved business operations.

9. Privacy and Confidentiality (HIPAA Compliance)

BOS LMT complies with the Health Insurance Portability and Accountability Act (HIPAA) to safeguard client health information.

• All records are stored in HIPAA-compliant systems (e.g., Kalix Health).
• PHI (Protected Health Information) will never be shared without written consent unless required by law.
• Clients may request copies of their records in writing.

10. Infection Control and Sanitation

BOS LMT follows CDC and State of Michigan infection-control standards.

• Each treatment room and tool is disinfected between clients.
• Practitioners use PPE, gloves, and hand hygiene.
• Clients exhibiting contagious illness symptoms may be refused service and rescheduled.

11. Transportation (Uber Health Addendum)

For select clients, BOS LMT may arrange HIPAA-compliant transportation through Uber Health or similar providers.

• Rides are limited to medically related appointments.
• No health details are disclosed to drivers.
• BOS LMT is not responsible for delays or incidents during transit.

12. Liability Waiver

By booking and receiving services, you:

• Voluntarily assume all risks associated with massage therapy.
• Release BOS LMT, its owners, contractors, and employees from any liability for injuries, allergic reactions, or adverse results.
• Agree that BOS LMT shall not be liable for incidental, indirect, or consequential damages of any kind.

13. Intellectual Property

All website materials, images, videos, and written content are the property of BOS LMT and protected under U.S. Copyright Law. Unauthorized use or reproduction is strictly prohibited.

14. Modifications to Terms

BOS LMT reserves the right to update, amend, or discontinue these Terms or any services at any time without notice. Updated terms take effect upon posting to the website.

15. Governing Law

These Terms and Conditions shall be governed by and construed in accordance with the laws of the State of Michigan. Jurisdiction shall lie exclusively in the courts of Wayne County, Michigan.

16. Acceptance of Terms

By booking, paying for, or attending an appointment, you acknowledge that you have read, understood, and agree to all policies contained in this document, including:

• Cancellation, No-Show, and Payment Policy
• Privacy and HIPAA Policy
• Liability Waiver

You further acknowledge that all services are final and non-refundable and that BOS LMT reserves the right to enforce these terms strictly.
